Is prophylactic antibiotic treatment justified in neonates?

چکیده
Infections are second leading cause of mortality preceded only by cardiovascular diseases.1 Neonates in particular are at a high-risk of developing bloodstream infections because of their immature immune system and mucosal surfaces.2,3 In neonates, sepsis has repeatedly been indicated to be an important cause of mortality, with preterm and very low birth weight (<1500g) infants being at the highest risk.3 Nosocomial bloodstream infections, are vital reasons for neonatal morbidity and mortality. According to a study 64% were caused due to grampositive organisms, 27% due to gram negative organisms and 8% due to fungi, the most common of the infectious agents being the coagulase negative staphylococci.4 The first microbial colonization in a newborn occurs during the passage through the birth canal and therefore the empiric treatment for early onset sepsis is based on the most common organisms detected at birth and that of late onset sepsis depends on presenting clinical signs.5

Prophylactic antibiotics to reduce morbidity and mortality in neonates with umbilical artery catheters.
BACKGROUND Umbilical artery catheters are often used in unwell neonates. Infection related to the use of these catheters may cause significant morbidity and mortality. The use of prophylactic antibiotics has been advocated for all newborns with umbilical artery catheters in order to reduce the risk of colonisation and acquired infection. Countering this is the possibility that harm, such as the...
متن کاملUse of Serial C-reactive Protein Measure-ments for Determination of the Length of Empiric Antibiotic Therapy in Suspected Neonatal Sepsis
Background: The management of neonatal sepsis especially in developing countries is problematic. There is no single reliable marker of infection available at the present. C-reactive protein (CRP) has long been used as a marker of infection. Serial measurements of CRP are recommended as a guide for duration of antibiotic therapy. Objective: To evaluate the serial CRP measurement as a guideline...

ABSTRACT Necrotizing enterocolitis is the most common acquired life threating intestinal disease in the neonates. This disease affects predominately preterm infants (less than 37 weeks gestational age). The pathogenesis of NEC is unknown predisposing. Risk factors for NEC include: lschemia, Hypotension, RDS (Respiratory distress syndrom), umbilical artery catheterization, Hypothermia...
متن کاملACOG Practice Bulletin No. 120: Use of prophylactic antibiotics in labor and delivery.
Effective prophylactic antibiotic agents are active against most but not all potential pathogens, which usually are endogenous flora. The goal of antibiotic prophylaxis is to prevent infection, not to cure or treat disease. In contrast to the therapeutic use of antibiotics, prophylaxis must be administered before the potential exposure, and usually for a short duration (less than 24 hours). The...
